Bonus TIPS for Successful Posting

Try a Search First
It is highly recommended that a Search be done on your topic before posting, as many questions have been answered in prior posts. As with any search engine, the shorter the search term, the more "hits" will be returned, but the more specific the search term is, the greater the relevance of those "hits". Searching for "table" might well return every message on the board while "tablesum" would greatly restrict the number of messages returned.

When you do post
First, make sure you are posting your question in the correct forum. For example, if you post an issue regarding Desktop applications on the Mobile & Browser Applications board , not only will your question not be seen by the appropriate audience, it may also be removed or relocated.

The more detail you provide about your problem or question, the more likely someone is to understand your request and be able to help. A sample database with a minimum of records (and its support files, zipped together) will make it much easier to diagnose issues with your application. Screen shots of error messages are especially helpful.

When explaining how to reproduce your problem, please be as detailed as possible. Describe every step, click-by-click and keypress-by-keypress. Otherwise when others try to duplicate your problem, they may do something slightly different and end up with different results.

A note about attachments
You may only attach one file to each message. Attachment file size is limited to 2MB. If you need to include several files, you may do so by zipping them into a single archive.

If you forgot to attach your files to your post, please do NOT create a new thread. Instead, reply to your original message and attach the file there.

When attaching screen shots, it is best to attach an image file (.BMP, .JPG, .GIF, .PNG, etc.) or a zip file of several images, as opposed to a Word document containing the screen shots. Because Word documents are prone to viruses, many message board users will not open your Word file, therefore limiting their ability to help you.

Similarly, if you are uploading a zipped archive, you should simply create a .ZIP file and not a self-extracting .EXE as many users will not run your EXE file.

Anyone know what is required or how to do electronic signatures?

    The title says it all.

Anyone know how to do this?

What represents an electronic signature for you? Text that says "signed by John"? Or need a signature image? What are you trying to sign and from where? A little more detail is needed.

      Hi Mike,

That's just it - I don't really know what is required.

What I want to do is to allow users to electronically sign a report or clinical note. I was hoping to get some pointers on: what is required (i.e., what is considered 'acceptable' legally), and some suggestions on how to implement it.

        Hi Gary,
I'm working currently on a document control application where one of the features is signing off on policies and procedures. The following thread got me in the right direction and should help.

        http://msgboard.alphasoftware.com/al...highlight=word

As far as legalities, the only thing I understand as being necessary is that the application structure that generates the signers name as a signature on a document occur within an application that has a secure, login-password doorway. That's not hard to setup. Attaching the signature wasn't hard. What I am finding difficult is that I want to give each document signed a unique document number and have this number attached to the document in the footer of the document. I have found this a bit more involved in a network environment, but I believe I am very close to completing this. By the regulatory standards that govern medical laboratories, a text statement at the end of the document as follows is sufficient:
"This document was read and approved by Mike Wilson, MD, on 03-29-2010"

        Good luck.

          Mike,

Thanks so much for this. So, if I understand correctly, the statement quoted above is an electronic signature? And, that just needs to be placed on the document?

Is there a requirement that once it is electronically signed, the program cannot allow it to be edited or deleted?

For official patient records, once signed, edits and changes are not permitted to the original record. If an edit/change is needed, it is required that the original signed document be maintained in its native signed state and either an addendum or a corrected component be generated. The addendum/corrected component can be a new document, or an addition to the existing document "at the end", so that the original signed document remains completely intact WITH NOTATION AN ADDENDUM-CORRECTION exists. I have seen it done both ways.... added at the end of original report, and a new report linked to the original report. Up to you.

Gary, I will share with you that if you engage in this... there are requirements that all patient electronic records must fall behind a formal, secure user login with password doorway and that every access to records have a logged audit trails of the user that accessed the patient record. Do you understand the complexity and build of this? Are you ready for that?

I know that Tom Cone worked at one time with Cheryl Lamiere (spelling ?) on a patient records system and worked on an audit trail component for that application. You might touch base with a personal email to him about this.

Legally Gary you are not allowed to store signatures electronically due to the fact the you could paste them into anywhere you wanted to. They wont hold up in legal court.

I understand digital signature as a private key that identifies the user from others.

I don't know if it's of any help as I am not totally clear on what you need, but when you setup a UK company online, your electronic signature is made up from the first three letters of three items: your mothers maiden name, your town of birth and your eye colour. For example: LAWson, BOSton and BROwn = LAWBOSBRO. You don't sign any forms as it's all done online.
